Eczemen: constitutioneel of atopisch

Er zijn heel veel verschillende vormen van ezeem. Een van de veel voorkomende vormen is het zogenaamde atopische eczeem.Dit wordt ook wel constitutioneel eczeem genoemd. Het is een chronische ontsteking van de huid, zonder dat we daarvan de oorzaak kennen. Jeuk staat centraal bij dit eczeem.


  

Voor constitutioneel eczeem worden vaak dieeten voorgesteld, maar daar is weinig bewijs voor. Wel kunnen er in individuele gevallen overgevoeligheden bestaan voor bijvoorbeeld kippe eiwit, maar dat moet dan door een dermatoloog vastgesteld worden. Men kan hiervoor geen bioresonantie gebruiken. De aandoening komt vaak voor, 16 per 1000 patiënten. 1 op de 3 patienten met dit eczeem krijgt later astma.

Constitutioneel eczeem is een huidaandoening met roodheid, blaasjes, korstjes, schilfers of schrale plekken. Jeuk is een essentieel kenmerk.

Constitutioneel eczeem komt vaak familiair voor en is één van de aandoeningen die past binnen het zogenoemde atopisch syndroom. Onder dit syndroom verstaat men de combinatie van astma, allergische aandoeningen van neus en oogslijmvliezen, voedselallergie en constitutioneel eczeem bij één persoon of binnen één familie. Er zijn aanwijzingen dat het atopisch syndroom een genetische basis heeft

Dieet bij atopisch eczeem

De Cochrane groep kwam tot de volgende conclusie:

There may be some benefit in using an egg-free diet in infants with suspected egg allergy who have positive specific IgE to eggs. Little evidence supports the use of various exclusion diets in unselected people with atopic eczema, but that may be because they were not allergic to those substances in the first place. Lack of any benefit may also be because the studies were too small and poorly reported. Future studies should be appropriately powered focusing on participants with a proven food allergy. In addition a distinction should be made between young children whose food allergies improve with time and older children/adults.

Voedsel allergie uitsluiten is vooral van belang bij jonge kinderen (tot 2 jaar). Ook recent werd de rol van zogenaamde eliminatie dieeten gerelativeerd. Het belangrijkste voorkomende aspect lijkt borstvoeding en het afzien van te veel ander soort voedsel in de eerste maanden.[1]

Coaching, psychologie 

Ook voor coaching en psychologische begeleiding bestaat weinig bewijs.

A lack of rigorously designed trials (excluding one recent German study) provides only limited evidence of the effectiveness of educational and psychological interventions in helping to manage the condition of children with atopic eczema. Evidence from included studies and also adult studies indicates that different service delivery models (multi-professional eczema school and nurse-led clinics) require further and comparative evaluation to examine their cost-effectiveness and suitability for different health systems.

Dit werd bevestigd in andere studies.[2]

Corticosteroiden 

Het meest worden corticosteroiden gebruikt om de ontsteking in de huid te remmen. Dit helpt vrij vaak, maar het is een symptoom onderdrukkende therapie. Andere stoffen, zoals pimecrolimus zijn minder effectief.

Prebiotica en Probiotica.

Over prebiotica, stoffen die de darmflora ondersteunen het volgende:

Reactions to foods and allergies (including asthma, eczema and hay fever) are common and may be increasing in developed countries. Many infants become sensitised to foods, including infant formula, through their gastrointestinal tract, a process that may be affected by the composition of the intestinal bacteria. Attempts to promote the growth of normal gastrointestinal bacteria and prevent sensitisation to foods have included the addition of prebiotics to infant formula. Prebiotics are nondigestible food components that help by selectively stimulating the growth or activity of ‘healthy’ bacteria in the colon. In formula fed infants at high risk of allergy, this review found one small trial that reported a mixture of prebiotic oligosaccharides added to infant formula reduced the incidence of eczema in infants up to six months of age. A second trial in low risk infants reported no difference in eczema. No consistent adverse effects were found on infant growth. Further studies are needed to confirm these results, and to determine if the effects persist beyond infancy or affect other manifestations of allergic disease.

Het lijkt er dus op dat prebiotica misschien zinvol zijn, maar dat staat nog niet als een huis. 

Voor probiotica is wat meer bewijs, maar hier is het zeker van belang er op te wijzen dat er allerlei verschillende porbiotica op de markt zijn, en dat deze conclusie niet geldt voor alle probiotica! [3]

Attempts to promote the growth of normal gastrointestinal bacteria and prevent sensitisation to foods have included the use of probiotics. Probiotics are live bacteria that colonize the gastrointestinal tract and provide a health benefit to the host. This review found that probiotics added to infant feeds may help prevent infant eczema, with one study suggesting the benefit may persist to four years of age.

Kruiden, homeopathie, visolie en andere alternatieven

Chinese kruiden zouden zinvol kunnen zijn, maar de producent moet absoluut betrouwbaar zijn, omdat bij diverse Chinese kruiden corticosteroiden worden bijgemengd..of andere stoffen. Een bepaald kruiden preparaat leek werkzaam, maar dat wordt helaas niet meer geproduceerd. In een recente studie bleek dat meer dan 40% van kinderen een alternatieve behandeling ondergingen naast de reguliere behandeling. Het ging dan hoofdzakelijk om homeopathie en kruiden. Spectaculaire effecten werden niet gezien. [4] Gewaarschuwd werd voor de bijwerkingen van sommige kruiden.

In een studie met meer dan 100 kinderen bleek een homeopatische behandeling vergelijkbaar te zijn met een reguliere behandeling, [5] Ook bleek een bepaalde homeopatische kruidencreme effectief in een open studie. (Reliéva cream) [6]

Een studiegroep uit California maakt melding van positieve effecten met bepaalde planten en granen:

Oatmeal, particularly avenanthramides, a newly discovered oat fraction, may be of particular value in restoring the cutaneous barrier and reducing symptoms of AD. Feverfew, licorice, and dexpanthenol also have been shown to be effective in the management of inflammation. [7]

Bepaalde oliesoorten verwant met visolie lijken veelbelovend, maar dat is nog niet zeker. [8] Andere studies naar de effecten van visolie waren negatief. [9]

Acupunctuurvormen kunnen ingezet worden om de jeuk te dempen, en werken door histamine te remmen. [10] [11] [12] Ook hypnotherapie kan zinvol zijn. [13]

Zinloze alternatieven 

Er zijn een aantal alternatieve behandelvormen die zinloos zijn, of aantoonbaar onwerkzaam. Bijvoorbeeld elektroacupunctuur volgens Voll, EAV, bioresonantie en injecties met eigen bloed. Deze behandelingen verhogen alleen de kosten van de gezondheidzorg en zouden gemeden moeten worden.
